How Physicians Can Order a Genetic Test Through Discovery DNA

Physicians can download the current, version-controlled fillable PDF requisition and the clinical information form.
Only the forms posted on our website are considered valid and up to date.
2.
Fill out all required fields directly in the PDF. Please save a copy of the completed requisition for your own records.
3.
Email the completed requisition and clinical information form to our secure lab email:
Submissions sent via any other method will not be accepted.

What Happens Next

You will receive a brief email confirmation once the requisition is received.

Discovery DNA will issue an invoice directly to the patient for payment.

Once payment is received, a sample collection kit will be shipped to the patient with prepaid return shipping.

Once the final result report is available it will be securely emailed to the ordering physician (via email provided at submission of requisition) for review with the patient.
